Anyone had this happen to their brake rotors?
Any ideas? Defect in materials?

If you let the car sit for long periods or park near the ocean, that can happen.
And they CAN'T fail you for that. They can only fail you if you're down to metal to metal on the pad/rotor surfaces. The rotor has to be broken or have obvious cracks/defects to warrant a failure.
That said, I'd change the rotors and pads if it was my car. Once you've got THAT much rust on the rotors, it will never wear off under normal usage.
