My friend's 1997 A4 1.8 Quattro, Tiprtonic had this problem back in Boston three years ago. At the time it was still under warranty and my recollection is that Audi ended up replacing the entire transmission. I drove it at the time and it was horrendous. I'm not sure they ever truly "diagnosed" the problem; they just replaced it. I'll ping him for more info.

I don't know what the root cause was, but the folks at Brookline VW/Audi ended up getting in touch with engineers in Germany, and ended up just replacing the transmission. Fortunately mine was under warranty at the time!

When clutches die they start slipping or you just can't shift into gears at all (without exact rev matching)
If you have a stock exhaust, you should shift sooner to be more in your power band.
The noise could be coming from your motor mounts, or somewhere in your driveline.
Rule #1.. if it seems like something's gonna break, stop doing what causes it till it's fixed.
Inspect your motor mounts, and install a snub mount. Don't shift too hard in an A4, it won't last long.
