Fan Clutch Failure

My wife's 1996 A4 had a fan clutch failure that caused complete disintegration of the fan blades, many broken plastic covers, a punctured radiator, a torn serpentine belt and a dent in the hood. The total cost to repare was $1218 plus a $200 towing bill. Interestingly one week earlier our neighbour's 2000 Allroad had the same failure. Has anyone else had the same problem?

How common do you feel this is. I am trying to get enough information that if this is a common defect to have Audi recall the vehicles with this problem and either replace the clutch fan with an electric fan or to provide an updated clutch fan that will not fail. The cost of the clutch fan was $143. The cost due to the failure was $1418 and this does not include the reduction in the value of the vehicle due to the dent in the hood.
