help needed ... questions about cam chain tensioner

my friend's 01.5tqms has been making a strange knocking sound from the back of the engine at idle when cold. When you bring the rpm up just a few 100rpm, it goes away. Once the engine reaches operating temp the sound is gone.
At first, we thought it was just noisier lifters, but recently, it's gotten a bit louder, and lasts longer. Does this condition damage the engine? If so is it even causing damage when you don't hear the noise?
How long have others gone with this noise, and did it do any damage to your engine?
She's supposed to drive to Yosemite this weekend, and I don't think she can get it fixed before then.

She needs to have the car looked at right now.
And "knocking" is not a lifter problem. Bad lifters make a tapping noise.

pretty sure its the second because the sound is from the back of the engine ... no sound from timing belt cover area.
