leak around coolant tank

leak around coolant tank
Checked my wife's car this weekend and found crud around the coolant tank. The tank seems to have been exposed to an extreme amount of heat because it was deformed, and I can clearly see a gap around the seal. I probably need to have the tank replaced and the system flushed at the same time. Anybody else had this experience? I just want to know what could have gone wrong.
