Leaking coolant...Engine temp sensor went up to H!!! NEED HELP

The coolant is boiling. I filled the coolant and no luck. Coolant temp just under 250 degrees and the car stays at normal then goes up to H after driving for about 5 minutes. NEED HELP. ANY IDEAS? 1998 A4 1.8TQ

Once the car began to overheat i stopped put more fluid in and it did it once again so i opened the cap and then coolant released from somewhere below the coolant holder.

The system is supposed to be pressurized. If you are removing the cap on a hot vehicle you should always have the engine running.
Its difficult to say where it's leaking from based on what you have said.
