New clutch slipping in 4th gear at full throttle (APR 1.0) - need help

I had a new (factory) clutch and flywheel installed ... a few thousand miles later I have a problem with the clutch slipping in 4th gear at full throttle (APR 1.0) around 3k-4k rpm ..

Any ideas as to what would cause this?

does it slip more the longer you drive?....

or is it always the same?
