Some help needed. Anyone ever seen a Kombi Valve adapter dumping straight exhaust into the air?

I was driving along the other day when all of the sudden my car sounds like an airplane. I thought for sure I had broken the flex joint downpipe connection. (It was a windy road and I was pouring it on hard through every corner.) It turns out there is direct exhaust dumping into the air from what appears to be a connector/frost plug in the Kombi valve adapter. Is there supposed to be a frost plug in there or did I blow a hose off of it?
I have done several searchs and can't seem to find a part number listing and/or diagram of the kombi(Combination Secondary Air injector) valve (the one back by the firewall). I would really appreciate it if someone could post the part numbers. Without them, there is no way in hell I am going to be able to even describe this to the stealership let alone a decent mechanic.

similar to a bypass valve. So that hose not being there won't cause a "leak" at your head. You just won't get the combi valve to open up for the smog pump to push air to the engine. The only place for a leak is between the combi valve and the head, or the combi valve and the smog pump plumbing. Both of those connections have gaskets, so perhaps one of those gaskets is leaking.

I finally got around to snapping some pics of my Kombi valve adapter. I had to throw it on myspace for now as I don't have an account elsewhere. Other pics will be in posts to follow.
As you can see there is a large Hole in what looks to be a plug location. Exhaust gas is dumping out of it at moderate to high manifold pessure and when the secondary air injector is running. Small amounts of exhaust come out a.k.a it is more quiet at idle once it has warmed up.
I am sure I will need to replace both the kombi valve and what ever this adapter thing is. I just need to find some part numbers.

Thanks for the reply post. This is actually coming off of the Kombi valve adapter at the bottom of the Kombi valve. It is a large (thumb size) hole with really stright un-muffelled, really loud, exhaust coming out of it.
I have already replaced the controlling vacumum line. That did quiet it down some under lower manifold pressures when the secondary injector isnt running.

it's a waste of a couple hundred dollars if you have a working combi. When you remove it, just apply vac to the nipple and make sure you can blow through the valve. With no vac on the nipple, make sure you CAN'T blow through the valve.
