How did I warp my rotors in 6,000 miles/brake dust question

Question 1: Audi just replaced pads/rotors up front due to a braking shudder cause by warped front rotors in my 03 A4 1.8t Avant Sportpackage. How did I manage this in 6,000 miles?!
Secondly: the rear wheels are always full of brake dust, the fronts generally way cleaner. Isn't this opposite of what one would normally expect?

If you use the search function you will find that generally on all our cars the rear wheels get dirtier much more so than our front. No one has really been able to explain this. It maybe caused by inferior brake pads on the rear wheels or just simple air dynamics.
