How does one actually read the code in the ECU?

Default No they do not write the code from the ground up....m....

They will download the code once they break the barriers (in h/w or s/w) set up to prevent it, and then decompile. Usually, they will not touch the executable code itself. Instead, they look at the calibation tables and start redeveloping the calibration in order to realize the performance gains they are after. In some cases, a modified calibration may trigger certain features in software to execute, and then they have to wok around these "failure management mode" features. The process is iterative and can take weeks or months before the new calibration is at a level to be released.
