I see folks complain about it but I haven't read an answer to your question yet.
I just had it done and I wasn't asked to sign any form as talked about below and the dealer is mod friendly.

Mine done Friday, seems to have less of the low end 2nd gear sluggishness. Also RPM's in 5th and 6th
