Brakes are "rumbling." Any ideas?

In the last few weeks my brakes started to make a low rumbling sound. Not a squeal and it does not seem to affect braking at all. It's just a rough friction sound. It sounds like something has gone bad between the pads and rotors.
Any thoughts on what's going on?

... in my Toyota Sienna minivan. The front brakes at times sounded like grinding (or rumbling), almost as if there was metal on metal.
I took it to Toyota for new brakes, and they said brakes are fine and that noise is common. The noise continued for another 8 months or so, and I finally took it back. The answer was the same, that the brakes are fine. (I can't believe I was asking/begging for them to instal new brakes, but they would not do it because it was not necessary.)
Finally 2 years after they first started making the noise, I had new pads installed. There was no damage or wear on the rotors. It was simply the noise that the pads made. Strange but true.

I've had that problem when I first start driving after my baby has sat for a while (especially overnight). In my experience, I think it is a cold weather issue....but I don't know if you are keeping your car inside or out....doesn't seem to effect stopping distance so I'm gonna wait till spring and see if my hypothesis is correct. Either that or I'm going to run some dynamic mechanical thermal analysis or the rotors, pads, and calipers.....I think waiting is the easier option
