16409 - Bank 2: Camshaft B (Exhaust): Advance Setpoint not Reached (Over-Retarded)
P0025 - 002 - Lower Limit Exceeded

So my question is, what do i need to do or replace to get this fixed.

Could be a lot of things. There is a procedure in the manual to test the cam advance system. There are four cams/four cam sensors. They are at the back of the valve covers, axial to the cams but at an angle. About $30 each at the dealer and a 5 minute job. I forget what bank B is, but I think it is on the driver's side. So it would be the cam sensor closest to the fender (exhaust) or the driver's side.
