what the heck is this?

there is a small grill on the drivers side b pillar in the middle of the pillar. Closer to the trunk release. I can't seem to figure out what this thing does.

back-up beeping sound. I have wondered about this thing also, in fact have lost much sleep. I thought I figured it out once, but I forgot. Or as ZCD says.

Both Brian Lay and Finman are right: it's the small speaker used in Bose cars to output the "external" audio from handsfree phone, nav, etc...
The Symphony head unit routes speaker-level output (from external sources like Phone and Nav) to one of the front speakers (IIRC, programmable from the same menu structure as SVC level).
HOWEVER, the Bose system has active speakers, so you get an itsy speaker in the B pillar, connected to one of the speaker-level outs on the Symphony head unit. (Left here in the USA.)
Anyone with an Audi phone (or "properly" wired aftermarket) and Bose can confirm that this little speaker is used by the phone.
