Turned out the gasket on the gas cap was not properly installed. Service tech adjusted it, told me not to worry.
If you look on the actual gas cap, there is a black gasket about 1/4 in thick. It rolled down about an inch, so the cap was not sealing.

Wow, after all the discussion about all the extra technology leading to extra fault codes, and the problem turned out to be a measly gasket.. just goes to show how important the small details still are!

How interesting, I was just putting gas in my car yesterday and noticed that the rubber gasket had come off and was stuck on the fuel intake nozzle. I bet this has something to do with the error code.
