I would hope that a 0.05 to 0.1% jack failure rate would get their attention, given that most people never even use the thing.

I agree.
I called Audi USA yetserday and they don't cre. They sent me tothe local dealer. I am now talking with their service manager.
I have filled out the form and suggest that everyone who has had this happen to them should also report it.

As an alternative, get an aluminum hydraulic jack from autozone or wherever. Place the jack under the rubber bumpers that are beneath the rocker panels when you need to raise car.
