Today I found the cause of my no mic issue and wanted to post my experience here as it may help someone down the road.
I recently replaced my failed bluetooth module with a used one off eBay. The original one had definitely failed as it would not function at all with just an error message in MMI Tel menu and the Address 77 Telephone controller was inaccessible in VCDS (though it did maintain the optical MOST loop). I swapped out the bluetooth module under the driver footwell myself thanks how-to's in older posts on this forum. The new module showed up in VCDS, the MMI Tel menus worked, and I was able to pair my phone but eventually discovered no one could hear me when using the car bluetooth to make/receive calls.
VCDS showed Address 77 Telephone had no errors or faults, Address 09 Central Electronics had correct coding as pointed out earlier in this thread, and I tested with two different iPhones (6 Plus and 6S) with same no mic results.
Finally, after comparing the current Address 77 Telephone coding against my earliest VCDS scan, I remembered I changed the coding right after replacement module install. I looked at the coding and saw that "Voice Control Language" was set to "Voice Control OFF" (coding 0010900) instead of "English USA" so I changed it to 0010970 incorrectly assuming that's what it should be.
I figured the "Voice Control OFF" coding was from the car the used module was sourced from; it was not and I should have left it 0010900. Once I changed the coding back, the mic worked along with everything else.
TL;DR: If you have no bluetooth mic on US D3 using known good phones and Address 09 Central Electronics is coded correctly, make sure Address 77 Telephone coding is 0010900 and not 0010970.

The voice control on the steering doesn't work since my Q7 didn't have navigation option... Message was "inactive button". I'd changed from 0010900 to 0010970 and the steering button works... My phone's still working fine both voice and speakers. May be I should check.

UPDATE: I found a thread on a UK A8 forum that gives reasoning for the counter intuitive 0010900 coding. That thread deals with loss of mic with bluetooth telephone module following the Euro 5570 MMI update but the same issue may have occurred with the latest or an earlier US MMI update.

If you have voice control, then the microphone is connected to the Voice control module since it is possible to have voice control and no bluetooth.
You code the phone module without voice control because the voice control module is in effect controlling the infotainment (including the phone as it is part of the infotainment).
If you code the phone module for voice control, then the phone module will expect the microphone to be connected directly to itself, and it in turn would then handle the voice control. As a side effect, since voice control is now listening through the bluetooth module without a microphone, the whole thing stops working.

Perhaps in your Q7 the bluetooth and voice control systems without navigation are interconnected differently and therefore the 0010970 coding works in a Q7?
