top part of driver's seat is adjustable forward and aft independent of regular recline motion of the seat. lumbar is up/down and inflated/deflated. everything is motorized

Does the lumbar section adjust rake, forward, aft? The lumbar adjusts with a 4-way "joystick" Only the 4 ways. There is another seat back pad adjustment independent of the entire seatback angle adjustment which probably accomplishes what you are asking about.

i'm finding i'm spending a lot of time in the passenger seat (having to travel a lot to hospitals out of town).
looks like it would be hard to find a used D3 with a pass. seat that has the extension.
the thigh support makes a big difference in the amount of pain that i'm going through on these trips.
tx. not so schnell lately.

It's not the W12 that I think matters here specifically, but any options needed and maybe differences in the model years. In my 2006, it has the ventilated seat option. Basically, the driver and front passenger seats have identical functionality, the only difference being only the driver's seat ties into the identification function for the particular key remote being used by the driver.
On the 4 seat question, while the thigh support does not move forward independently like it does in the front seats, as you recline the seat the whole seat bottom cushion is sliding forward (via a motor) which is then increasing the back angle at the same time.

i may be looking at D3s...
so far BMW is offering the seat I want in either 5 or 7.
there are some decent 05's coming in off of lease CPO....looks like some savings to be had if audi keeps its interest rates low.
