I believe the OP is addressing what he hears through the car's speakers versus what the other party is hearing on the other end of the call. Not a microphone issue, but something wrong with reception clarity.

Have you thought about...the phone?
In my experience, 90% or more of bluetooth issues where the car is the initial suspect that folks post up end up being something with the phone. Phones get dropped and 100 other things regularly, including coincidentally perhaps about the time of getting a new car.
When in doubt, try a known good phone in your car, and your phone in someone else's car.
BTDT myself, when i realized the two day old iPhone Apple gave me (after a bad camera prior one) then had faulty bluetooth where it didn't work w/ the car's microphone on hands free at all. But...her phone did...so it became quickly obvious it was the phone, not her Q5 or my D3 W12 where I first noticed it. It also worked fine as a stand alone phone (outside of car or if I took it off bluetooth or used an earpiece). Thus, second car with same issue and second phone working fine were telling me pretty obviously it was the phone, not the car. Never mind Apple's quasi flagship store in Palo Alto had trouble understanding that in diagnosing phone bluetooth, you don't connect it A2DP to a music player and declare all is fine (not). Got to explain to the "Geniuses" how they aren't the same. Second refurb phone under warranty and all returned to normal.

I have this same issue. I have a 2013 A8L and an ATT powered iphone 5s. The quality of the audio I hear is terrible. If it's a higher pitched voice (woman or a young kid) it's even worse. The speaker sounds like it's almost blown. Music sounds fine (I have the Bose system) when I brought it in the technician just kind of shrugged and said he's heard the complaint but it's not something they can fix. I'd love to get some feedback on this as well.

I don't have the problem. I have a Verizon iPhone 6 Plus in either my '14 RS7 with the B&O or the '15 A8L with Bose.

Per my earlier post, it was indeed the iPhone 5S that was replaced three times by Apple under warranty. Used w/ AT&T. 1. Camera apparently due to internal physical distortion pulling apart in optics area; 2. Bluetooth (w/ faulty diagnostics by Apple store until I pressed it home)--bad out of box on refurb from #1; 3. Core electronics controlling SIM access rendering phone and network access (except WiFi) useless--first tried a SIM replace w/out success. Not impressed w/ Apple QC to say the least, and none were immediately following a drop or some such. Apple did replace each time w/out cost under warranty or AppleCare once they (somewhat painfully) diagnosed.
Again, don't assume it's the car unless phone is confirmed good via a different car attach, or a different known good phone attached to D4 showing same issues.

Just confirming that a proper functioning iPhone 5s plays nicely with the D4's internals. Obviously a phone that is out of sorts can/will louse things up.
