91 100 Quattro alt. belt problem - need help

due to something that is ruiing out of true. The alternator spindle may be bent or one of the mounting brackets broken.
It could be the idler pulley is beginning to seize up when hot?
Does the belt just come off and remain in tact or does it actually break?

Are you using the correct size Belt?
The 5 Cyl. engine is pretty particular about belt size. A local Auto Parts Store may sell you what the computer tells them is the "right belt", but that doesn't mean that your car sees it that way.
Get a belt from your Audi Dealer, or buy a Conti Belt from an online source.
I'd be willing to bet that your problem will be solved once the correct belt is seated in the pulleys.

is sticking and not maintaining enough tension on engine over-run when for a few seconds it is possible for the intertia in the alternator actually drives the belt and then it will become loose and fly off.

The I-5 engine uses three V-Belts for its accessories. The alternater uses a "Tooth and Rack" assembly to statically tension the belt.

Umm, Fan drive belt? Not on an I-5. The radiator is offset to the driver side of the car. It's electric.
I don't want to appear judgmental, but I sense that you have an obsession with Spring-loaded tensioners.
