I think maybe the coolant tank cap might have a faulty seal because it seems to be boiling over. I also noticed that the return hose to the head looks a little bulging. Help is appreciated.

Is the radiator fan kicking on? Are all hose clamps tight? Any visible leaks?
Here is a link I found that I responded to someone a while back. I was the queen of over heating issues. Sucked! And when I was in law school, it was the reason I left at 7am...so I could beat rush hour traffic so as not to over heat!

Your leaking tank, and bulging hoses sound more like symptoms of your problem.
I've had all my overheating problems caused by the rad temp switch failing.
Symptoms? Rad fan NEVER comes on, even when everything is toasty.
Easy test? Pull the connector off the rad switch (bottom rear of rad. Should be able to get to it easy, unless you still have your rad shroud intact! Not likely, as I've only seen that intact on one 4000, ever!)
Once the connector is off the rad switch, short out the two terminals together with a short piece of wire, or whatever.
Whenever these terminals are hooked together (and the ignition is on), the fan should run.
If not, could be the fan relay, or something to do with the fan itself (THe fan connector IS connected, and not all crusty, I'd hope?)
If that all works, then maybe look to the thermostat or something..

On top of the radiator...theres a bleed screw (bolt) whenever my coolant temp on my dash is going hot, I check my coolant level, fill if needed, then I open the bleed screw a lil, not the whole way off tho, and I let the car idle for 15 min with the heat on full blast, then when I start to see a lil bit of coolant leaking out the top of the radiator I tighten the screw and I either shut my car off or go for a drive. This gets all the air pockets out of the coolant system. hope this helps.

It was simply the fan relay plug at the bottom of the radiator. it was a little loose from all the bushes I was running over. Hopefully this fixes the problem. I was wondering why the fan wasnt running.
