Left Rear Wheel Wobbles at 45 mph...

It was vibrating the car at 75 mph and up before but I swapped the front tire with the back and now it happens at 45 mph. The tire that was on the back before looks to be out of round but probably only because of the wobbling.
Could this be caused by bad wheel bearings? I took the hub off today, but I really didn't know what I was looking for. After removing the nut, the outer wheel bearing just fell out. Isn't it supposed to be pressed in?
This (and rear shocks and ball joints going bad) came about after the 4500 mile trip to CA and back to ND taken over spring break. Must have rode her too hard. Drove around 100 mph about 90% of the time and no tickets or accidents!!!
