which parts need replacing? obviously the clutch disc and release bearing but what about the pressure plate and the pilot bearing?

Problem is you wont really know until you get down there! ;O)
Most parts houses sell a "Clutch Kit"! Which includes disk, plate, and bearing. Its recommended to change the pilot bearing.
Also the rear main crank seal, if its leaking.
Flywheel may need to be resurfaced?
