I got the car back yesterday after Step #2 ECM Flash (which I thought previously was to be a more comprehensive cleaning of the fuel system).
I did notice that on my work order receipt, the tech had noted "random misfires in cylinders #3 and #6".
I'll let you know what happens when I start it up later today.

Following the ECM flash, the car still has the rough idle during startup, but I have not noticed the knocks and thumps as before. I think there was an improvement but I would have to compare it to another S5 (cold start) to really see if this was fixed.
I'm going to monitor this for a while and see if anything changes.
Anyone else with this problem find that the ECM flash helped?

It started to appear on my 43k. miles 2009 A4 2.0T Quattro.
Never happened before since I've always used one gas station (shell) I'm assuming it's a gas issue since I drove from SF to LA and had to fill in LA's Chevron. So far happened twise on cold start.

I just called a dealership and the service advisor said that it has been a known issue (warrantee fix) with their cars, especially the S5. She said that the engines seem to be very sensitive to the ethanol mixture in fuel and suggested I (continue) to only use high-quality 93 (I normally fill at Sunoco). She said this happens at a cold start, and the Audi engine warms up very quickly, which is why after 10 seconds or so, the idle is smooth again.

I noticed a similar thing happen on tuesday while sitting in traffic. The engine was idling at 800 rpm and the car was shaking violently. I could feel it in my seat, I wasn't sure what was causing it. I assumed there may be a loose engine mount or something along those lines. after driving a few minutes it stopped. It only happened when at a stop and idling. I guess I will have to note it at my next service.

I drove back from LA to SanFran and luckily this trip used 11 gallons of that ****ty chevron gas that caused shaking earlier (twise on cold start) so, after arriving to my neighborhood I used "my Shell" gas station. Next morning, cold start, no issues, smooth as it was before. So I would say stick to one brand of gas. Seems my car doesn't like gas change.

Replace your spark plugs. DO NOT USE ANYTHING ELSE, BUT NGK plugs. There is a huge difference between NGK vs other vendors for Audi.
