CEL Twice in a month - DTCs 1136 and 1138

I've gotten these codes twice in a month: 1136:Long Term Fuel Trim Add.Fuel,Bank1 System too Lean and 1138:Long Term Fuel Trim Add.Fuel,Bank2 System too Lean. Any body had this before. Inday said it could be many different things. Said wait for third time. This is the engine they just did a full rebuild on two months ago or so.

log your MAF. I had a similar issue last year, had a bad MAF and loose RH IC intake hose. The hose fixed the CHK codes..

Since it's additive (add) it's likely not an air leak (intake-wise). MAF or O2's would be possible but probably not likely on your car. Best options would be DV's, DV lines, or the check valve feeding the DV's.
My first bet would be the DV check valve considering all the work done. Next would be the line slipped off the DV nipple (lame!). See below for check valve location....
