Multiple Broken Clutch Fan Blades

I have a 2004 Audi Allroad 2.7t With 175k Miles. A few weeks ago I had the clutch fan blade disintegrate and shredded the radiator. I replaced the fan blade and the radiator and all seemed well. I have been driving it for about a week and low and behold tonight the new fan blade broke apart a shredded the new radiator
When reassembling all clearances and torques seemed to check out just fine and I have been driving it for a week so I wouldn't think it would be an issue with clearance. The car has seemed to be having lower fuel economy and the fan seems to be on more than it should, but never overheating. Would a faulty clutch fan cause this? Anyone else experience or hear of this issue? The only other thing I can think of is that it is out of balance causing the fan to come apart like that. I really don't want to replace the radiator again to have the fan come apart again and put another hole in a new radiator. Any help or input would be greatly appreciated! Thanks guys!

The solution to Allroad fan failures is to replace the ill-fated fan design with the fan for an S4. It's in the archives here on AudiWorld.
