something wrong with my fan clutch?

When I accelerate (or just depress gas in neutral at standstill) the engine fan is speeding up and slowing down depending on the RPMs of the engine (making a very noticable noise).
Is this caused by fan clutch gone bad? If so, is it hard to replace? Costly?
If not the fan clutch, any thoughts?

It is a thin wall wrench that you hit with a hammer to knock the fan clutch loose from the pulley. I believe if I remember correctly that they have left hand threads I know that BMW's do but I always seem to forget whether the Audis are left or right. Maybe someone else can verify that part.
