NAR = North America Region. The site also shows the owners location as Pennsylvania.

I want a picture of their spare tire / trunk well! What would we see? an enormous 12 volt battery in addition to the other 12 volt battery that is in the footwell? or nothing?

This is my SQ8. It's a 2020 model year, North American model, with a 6/2020 production date. I do have the mild hybrid 48 volt system but have not experienced any electrical issues and I'm currently at 30K miles.

Looking on the VIN this is US car.
He doesn't have sway stabilization and rear axle steering. That is probably why there is no need for 48V. Engine starter is powered by 12V installation.

car probably has the denso air cooled alternator. so no alternator issues for you!

That explains why these features cost so much to add, it adds the 48 volt electrical system too.

Correction. I see no evidence of a mild hybrid system on the vehicle. I have no battery in the trunk/cargo area were the spare tire is located.

Edit: According to the factory sticker I do have all wheel steering.
