What is the format for submitting an article?

Writing a little thing up on the ND show. Do I put the URLs to the images in the story or is that separate.

Plain text or html with attached pics with notation of which pic goes where is prefered (like insert pic nnn123.jpg here).

Word docs or .pdf with embedded pics least desirable.
