What insurance companies only ask for 3 years of history??

How does that work? Do they agree they owe you?
Call them and work your way up to the highest level supervisor you can. Then let them know they have 48 hours to pay or you'll contact the state Attorney General's office and file a fraud complaint, and also look into a Class Action lawsuit. That'll make them pay attention.
