I'm making progress.....I think. I posted my scan below. Important correction to the information above: Since it's the slave module it's the driver side in question.

With this in mind that it's the driver side that is deviating 7.5 degrees from the initial calibration this actually could make sense (maybe); because about a year ago my wife backed into something that barely did any damage to the bumper but since then I noticed a slight gap between the bumper and the rear panel indicating in fact that it is probably hanging just slightly on that side. Not enough to notice when you look at the car but in fact 7.5 degrees isn't that much, and there would be some deviation there anyway, so this may have pushed it over the threshold........ so this is my current hypothesis. Only outstanding question is why it worked just fine for the last year... but possible answer to that is maybe after the "accident" it was close to the deviation threshold and is now over it as time as passed and the bumper has possibly sagged another degree or two.

So, I'm not in a rush but I will get the bumper fixed if it's as easy as just getting a new bracket or something not completely insane and will report back.

Inspected the driver side module and I believe I have found a smoking gun. There is damage to the bracket that secures the module to the bumper.

Did you end up getting yours to work? My wife’s 2015 annoyingly started doing this. Same module, no bumper damage, random. I also have VCDS.

Yes. After the bracket was replaced I was able to reset the error using VCDS and can use lane assist again. One thing to note though is it took some driving though... at first every time I hit the lane assist button after clearing the error it would beep and pop-up the error on the console saying Drive Assist failed. Then I reset it using VCDS and didn't hit the button to try again.... the error was cleared and I drove around not thinking about it and then tried the button the next day and, sure enough, it worked and turned on like it always did working great. Like Dennis said above, you can reset the error and if the sensor isn't out of position it should fix itself after driving around for a while which it needs I guess to do the recalibration/initialization.

Thanks for the reply. I’ll check for bumper/bracket damage that might be hard to see from normal viewing positions. I did reset it with VCDS so I’ll wait to test the button as well just in case.
Assuming worst case scenario, is it at all straightforward to code a new (used) module? On my BMW it’s not, but I’m a total VCDS newb.

Did you check your angle offsets like I show in post 20?

replacing the module won’t help unless the module itself has an error but there’s a separate error message for bad control module. My error never said the control module was bad (believe the error is that it can’t communicate with it if it’s “bad”)

Good point.
Just checked mine and they’re funnily enough very close to yours. 3.11 and -7.61.

Is your photo taken with the bumper off?

My photo is bumper on, laying on my back with a iphone

The -7.61 is your problem. The error message tells you if it's master or slave. So find out which side the -7.61 is and get under there to see if you see some bracket damage. My wife backed into something and barely did any damage to the bumper so it may not take much to knock these sensors off a few degrees.

Master = control module 1 = passenger side = Dynamic angle offset lane change assist 1
Slave = control module 2 = driver side = Dynamic angle offset lane change assist 2

Bummer. I found the angle that you used to view the sensors. Fortunately/unfortunately I don’t see any bracket damage at all. I’m not sure why it would go out of alignment unless somehow something hit the bumper and didn’t cause paint damage or maybe the sensor/ module itself failed internally. I looked at a video with what is required to calibrate the crazy thing. Pretty intense. We will likely just be without, sadly. At least there isn’t an annoying dash light or anything in the MMI.
