To blame solely your car and its brakes seems a bit harsh to me. We can only speculate as to why this happened but perhaps the road conditions played a role into it.
Perhaps you should try to reproduce an emergency brake situation on a road you know and see if the problem re-occurs. If this situation doesn't re-occur after a few attempts at an emergency braking test then it is safe to say that perhaps another factor was involved.
