I think I got it. Is it the locking perch nut below the perch itself?

I assumed a nut but it screws directly into the bottom mounting plate and by turning to adjust the height it can become loose. In the photo the two dampers on the right are the front (the ones that filed on my car) the silver plate under the red spring is where the damper rod screws in. This is where the rod threads that screw into the plate stripped away and separated both times. When you adjust the height at the top of the spring the whole unit can turn and unscrew the rod from the plate at the bottom. I am not a mechanic, but based on my discussion with the Audi mechanic and the discussion with Jason, this is what we believe probably happened.

So are you referring to the hex bolt located at the bottom of the suspension inside the "Y" end?

Look at the dampers with the red circle. At the opposite end of the u shaped bar is a silver metal plate. On the other side of the silver metal plate is where the silver bar of the damper mounts. It is this bar that is separating from the plate and stripping the threads in the process.
