2004 4.2 rough idle when warm, silky smooth for 5 sec after clearing codes

Hey there, I am new here and thank you for any and all tips!
So I read a ton of the older posts, everything I read I should be freaking out about chains letting loose soon. The reason I think this is odd / wrong is right after clearing the codes the engine has a perfectly smooth idle. more info on what has been done / tested.
The car has 130k miles, I have had it for 2 years and put on about 20k miles, no clue the history before, I am hoping the chain tensioner was already done as clutch and other things were done.
The car starts and runs fine / normal idle when cold, once warned up the jacked up idle starts. I know some would say its because cold start is higher rpm so tension on the chain, its still well under 1k rpm and once warm / the bad idle starts it takes >1k to get it to go away. Other than the idle once warm the car runs well all the way to redline.
Swapped plugs, coilpacks and checked for vac leaks, unpluged mas airflow to see if anything changed (it didn't) I did notice the crankcase seemed to be under fairly high vacuum so I will grab a new pcv.
I have a vag-com cable on the way, anything I can scan over / will clearly show bad chains or other things to look at?

So everyone likes videos!
Don't mind the vac line, I have been mapping out / fixing some of them. You can see / hear two check engine clears. The first few minutes on a cold morning the idle is fine until it warms up.
