Anyone else have problems downshifting into 3rd (grinding) . . .(updated)

or upshifting at speed (i.e. hard launch or drags)??? I'm almost afraid to ds into 3rd because 7 of 10 times, I grind the gears. I've been driving stick for almost 10 years, btw. It mainly happens on the downshift, but sometimes on the upshift. Any suggestions??? (and yes I rev match)
Update:
Got 2 opinions on my situation from my mechanic and another source. Two potential problems: (1) Synchro problem (2) low on tranny oil. Both of them gave me the same two optoins. More thoughts?

not happened to me so far (32K) miles. If I am donwshifting from 5th, I run the shifter through 4th without releasing the clutch before going into 3rd, rev matching through each gate.
When upshifting, I always try to pause through neutral, even for just a moment.

I took it into Audi dealer and they said it's a problem they've seen often enough in S4's. Do you have a short shift? If so, replace it with your stock shifter and make sure the neutral positioning is as close to stock as possible. Bring it in and they should repair or replace under warranty. Unfortunately when I removed my short shift, the neutral position was off. The dealer suspected foul play and charged me $250 to reposition the shifter and voided my tranny warranty. So I'm kind of S.O.L..........

have yet to grind any gear. I only match revs unless I am driving REALLY hard. Not that I need to, but usally when I'm down shifting I will X2 clutch. It's a force of habit from my damn truck that doesn't shift unless you X2 clutch.

I doubt it if there's a huge abundance of trans's failing. I thought Audi/VW made pretty much bulletproof drivetrains..My dad is still running the same 3-speed Auto on his '86 Jetta with around 120k or so on it.
