Has anyone had a Timing Belt tentioner (hand grenade) fail?...I did

the motor was making all kinds of sound....i took of the front TB cover to find the bottom end had all kinds of slop in it. Making all kinds of noise.
Broke down the fron end tp get access.....to find the grenade failed.
Luckily, the belt stayed tight on top and on the drive side, so timing was never lost.
Just another 6 hours and $100

My packing slip showed I bought a replaceent timing belt tentioner (hand grenade), and I remember seeing it. But when it came time to put it in the car, it was gone. I am thinking I may have thrown it out with the packing materials. No big deal, uppon further inspection...the original one looked fine, so I reused it.
last week, driving home from work, I jeard a rumbleing noise from under the hood at idle. When I had rpm's it went away. I got home and when I pulled into the garage, it was louder than I thought. I pulled off the timing belt gaurds, and the TB was loose and slapping against the guard.
I drove my other car for the last week, finially tore down the front of the car last night....only 2 hours this time...
...when I got the car to TDC...I was able to put the cam lock bar on with no issues...so i never lost timing...thank god. Took the crank pulley off, and saw the hand grenade was compressed, but no pin in it. so there was no tention on the lower timing belt. i took it off, hit it with a hammer..and it sprung up.....I compressed it in a vice...sometimes it sprung up...sometimes it stuck in the down position.
So, i went to the dealer today and got a new one...and will put the car together tonight. I am not sure why it failed....it took me 3 days to do the Tb service before and i had the pin in for a while..dont know if that was it or not...
The moral of the story....dont reuse the hand grenade...replace it.
