Here's a good pic, but an OEM pressure plate.....
I'm not entirely agreed on this as the cause though.

It could be that your shift fork is out of place, meaning your slave is misaligned and just hyper-extends until you blow it apart. Or you're just missing the shift fork altogether. Put some hair around it...
