Car has more than enough torque to start from a dead stop in 2nd without straining the clutch. The difference between 1-2mph and "idle" speed in 2nd is so minor, its no different to the clutch than starting from a stop in first...

I doubt there are many people who can shift well enough to safely put an S4 into 1st while moving. I know better than to do it... because "working" and not damaging things are two very different things.

That's a first gear feat. So what happens in this situation when you find yourself going from >5mph and then below it, often repeatedly? You either find a way to shift into first, or you find a way to afford that new clutch.

But there are often times when there is no other real option. I'll take my chances with the 1st gear syncro and save my clutch for another day. It's not like this is a problem unique to Audis or S4s - MANY cars are like this.
