Throttle Adaption - Do you get a fault code after doing one with VAG COM ?
And getting OK with the Block 60 ?
I don't have any fault code before I do the Throttle adaption but after doing one, if I check fault code, I get an error ( don't really remember which one, something like limit exceeded )

It can also fail if its too cold...
I've had that happen and throw a DTC. Had to start the car and run it for a few minutes and then do the throttle adaption. This only happens in the winter when the car has been sitting overnight though.
