remove caliper. pull to remove rotor.

There are too large hex head bolts... I forget the size... a BFH might come in handy too.

Are there any nuts or bolts that must be removed before the rotor will come off?

just the ones holding the caliper on, iirc. maybe 18mm??? the lug bolts hold teh rotor on.

Are the hex head bolts in the center of the rotor?

18mm bolts and they are usually very tight, i.e. you will need a breaker bar and some muscle

Are they standard bolts? I thought they were hex head... and Yes, they're VERY tight...

by hex-head you mean allen bolts? pretty sure they are standard bolts (use std socket).
