When draining the coolant, does everyone replace the O-Ring at the lower radiator connection?

Just did my T-Belt and a new snub mount. Flushed the coolant as well. Car fired right up and all was good, no apparent leaks.
I get in this morning, start her up and get the low coolant warning so i shut her off and pop the hood. There is nothing in the resevoir (I put 6L of 50/50 mix in)! So I notice small puddle under where the lower radiator connection is, no puddles anywhere else.
That connection is a pain so maybe I didn't get it all the way on (will check tomorrow morning). The other option I suppose is the O-Ring. Anyone else had a failure with it? I searched the archives and couldn't really find much.
TIA

I found it helped to lube the o-ring first with straight coolant.
