An 18" rim with a 45mm offset does what to the setup? Wheel is further out or in?

Depends on the width
Offset is the positioning of the wheel based on the width.
For a given width, a higher offset sits inward more. However with an offset alone, its impossible to say how the fitment will be.

I have 18x7.5x45mm wheels on my TT. They sit fairly flush with fender, but I used spacers
<center><img src="http://www.jobfax.ca/malish/PICT0004.JPG"></center><p>20mm front, 25mm rear. They fit very nicely
