Default Car running lean... due to Secondary Air Injection

I am receiving the following codes:

1.) 16795 / p0411 (incorrect air flow)
2.) 17536 / P1128 (fuel bank 1 lean)
3.) 17536

I have taken my car to the dealer twice and they haven't done ****.. it seems they just reset the light and send me on my way.. until I call them back again!

Does anyone have a good solution to this issue? Pics? From what I understand from the forums, DV might be one of the issues. Does anyone have some pics on where the D. valve?

Thanks in advance

This *could* be symptomatic of a bad MAF also.
